Toolkit for improving cardiac rehabilitation and heart failure services

This toolkit is designed to support health professionals with an action plan of how to improve services and two factsheets outlining the evidence for and benefits of cardiac rehabilitation and heart failure services. The Heart Foundation has developed this toolkit with the support of the Statewide Cardiac Clinical Network and the AMA Queensland.
