This tool is a conversation guide to help health professionals yarn about heart health and how to reduce the risk of heart attack and stroke.

The tool has been co-created with Aboriginal and Torres Strait Islander Health Practitioners/Workers and with community members. It aims to facilitate shared decision making about heart disease prevention in ways that support the health and wellbeing of Aboriginal and Torres Strait Islander communities.

Shared decision making means taking individual values and preferences into account, along with the evidence, to come to a shared decision. It is part of the national clinical standards. The design of this tool is based on a shared decision making model that was developed with Aboriginal and Torres Strait Islander communities, called Finding Your Way.

The tool is designed to be used in conjunction with a Heart Health Check based on the guidelines at cvdcheck.org.au. Once you know the level of risk, you can use this tool to guide decision making about what to do next to reduce the risk of heart disease.
