This directory presents information on cardiac rehab programs offered across Australia.

Cardiac rehabilitation helps your recovery after a heart procedure or the diagnosis of a heart condition. It also helps lower your chances of having heart problems in the future.

Cardiac rehabilitation programs include support, exercise and education that’s personalised to strengthen your heart. It helps you make long-term lifestyle changes so you can live a longer, healthier life. 

The directory also has a list of Heart Failure programs where available, for specialist support.

Cardiac rehabilitation via telehealth services

There are several organisations that provide cardiac rehabilitation via telehealth. Cardiac rehabilitation telehealth is where rehabilitation services are provided over the phone or by video conferencing. The list below includes some major telehealth services that are available across the nation.

Description: Cardihab is a clinically validated nationwide telehealth cardiac rehabilitation service provider. This service offers smartphone and tablet apps that connect to a secure clinician portal to support face-to-face, hybrid and remote delivery models.

Description: Medibank Heart Health at Home is a phone-based cardiac rehabilitation program that runs over 6 to 8 weeks and you can participate no matter where you live in Australia (eligibility criteria may apply).

My Heart, My Life support program

If you have recently experienced a heart attack or angina diagnosis, please consider joining our My Heart, My Life support program.

This program is free to join, and provides a package of support from the Heart Foundation including booklets, emails and text messages, to keep you on track.

The program does not replace you going to a cardiac rehabilitation program, but helps boost your recovery journey.
